ColumbiaDoctors began using the CROWN (Clinical Records On-line Web Network) electronic health record in 2008. With more than 1,200 providers, customizing the electronic record for each specialty/subspecialty was an extensive process, and the conversion from paper took place practice by practice. As of today, all departments except one use CROWN, which ensures safe and systemwide coordinated care for our patients.
The federal government’s "meaningful use" program provides incentives for physicians to incorporate electronic health records into their practice. This is to increase healthcare efficiency, reduce costs, and improve quality and outcomes. For further information, visit Centers for Medicare & Medicaid Services.
Soon you will benefit from our online patient portal (a link to MyColumbiaDoctors.com from our website), where your secure messages, prescription renewals, online forms, and test results will link directly to your electronic record.
For specific record requests, please contact your provider’s office. Click here (English Form / Spanish Form) to download an “Authorization to Release Medical Records” form, which is required to release your information
