Access Records via Connect
Easily request and view your medical records, test results, clinical notes (including lab, pathology, radiology, progress notes), and more through Connect—available to all ColumbiaDoctors patients per 21st Century Cures Act since July 2022.

If you prefer not to use Connect, you may request it digitally via an online form or mail in/fax a paper form (PDF).
