Medical Records
Columbia Orthopedics instituted the Clinical Records Online Web Network (CROWN) in 2009 throughout our orthopedic network, allowing clinical teams to access important patient information instantly at any of our office locations throughout the New York metropolitan area. If your treatment requires a team of experts, including any NewYork-Presbyterian Hospital affiliated physicians, your information can be shared seamlessly and securely. Please contact our Medical Records team if you have questions regarding the release of patient information.
Requesting a Copy of Your Records
If you need to request a copy of your medical records, please complete the Authorization for Release of Medical Information Form and return it to us via email or mail.
We will have your records ready and mailed to you within 10 business days of receipt of your request.
