Appointment Checklist

To ensure your appointment with us will be a speedy and pleasant one, please bring the following with you to your first visit:

  • Your completed new patient paperwork
  • All relevant medical records, including surgical reports and test results
  • Copies of radiological reports (X-rays) on compact discs (CDs) or films
  • Any medications you are now taking, including over-the-counter drugs and herbal supplements (Please bring the actual bottles or containers)
  • Your health insurance card(s)
  • A valid Driver’s License, state-issued ID card, passport, or other form of identification
  • An insurance referral (if required by your insurance carrier)
  • A list of questions about your health condition and treatment options
  • If you need assistance with completing any of the forms, our staff will be happy to help you and can be reached at appointments-ortho@columbia.edu or 212-305-4565.

Cancelling an Appointment

At Columbia Orthopedics, our goal is to provide quality care in a timely manner. Effective Jan. 1, 2025, Columbia Orthopedics has updated our cancellation and no-show policy. This policy enables us to more effectively manage available appointments and provide the best medical care possible.

  • Please be courteous and call us if you are unable to show up for an appointment so we can offer the slot to another patient.
  • Current patients must contact the office at least 24 hours in advance to cancel an appointment. If your appointment is not canceled within 24 hours of your scheduled appointment time, you will be considered a “no-show.”

No Show Policy

After the first no-show in our department, we reserve the right to charge for missed or cancelled appointments without the 24-hour notice. Failure to cancel at least 24 hours in advance or not showing up will result in a charge of $50 per visit. If you have a balance, future appointments may be affected, and numerous no-shows may result in the patient being discharged from our practice.

Note: Cancellation/no-show fees are ineligible for insurance or managed care plan coverage. They are the sole responsibility of the patient/guardian.

Signing up for Connect is easy. Ask about joining Connect at your next visit.  If you already have an invitation and activation code, visit our sign-up page. If you do not have an activation code, visit our self-sign-up page and follow the directions.

Connect is a joint effort of Columbia, NewYork-Presbyterian, and Weill Cornell Medicine, so a single Connect account links you to all three institutions.
